No. The other one seems still relatively new and fine, and this one is unnecessary and contains certain things, such as age, which should not be given into the format. That should be provided at the discretion of the person applying, if and only if they'd like to, as some people are automatically accepted or denied based on age, and people hold biases toward age groups, good or bad. If the age is made to judge the maturity of the player, allow the quality of their writing, their content to determine that. If it's made to judge the maturity, monitor their behavior ingame, say, when a hacker attacks them or when someone angers them. Don't judge it by a simple number
